Our patients benefit from our palliative care during any stage of their illness. They receive care along with any curative treatment. Care is not dependent on prognosis, and it is not restricted to end-of-life care.
Support includes:
- Assessment and relief of distressing symptoms.
- Discussion of treatment options with patients and family members.
- Addressing goals for chronic medical issues.
- Services can be provided at home, in an assisted living, retirement home, skilled nursing center/rehab, long term care home, residential home or a family residence.
- Providing guidance with advance directives and/or advance care planning while having goals of care discussion.
